New Patient Registration:
The Pines Surgery doctors welcome new patients who live within our practice area.
For each applicant applying to register you will need to provide the following:
- Proof of identity (e.g. Passport, photo driving licence or birth certificate)
- Proof of address - Two Utility Bills are required
- Proof of residency status (patients from overseas only)
Our Practice Area Covers:
Kingsthorpe, Kingsheath, Dallington, Spencer Estate up to Spencer Bridge Road, Semilong to Grafton Street, North of Campbell Square, North of Wellingborough Road, West of Lumbertubs Way, West of Thorpeville, Chapel Brampton, Church Brampton, Pitsford and Boughton.
All applicants that are accepted to the surgery, must complete:
- GMS1 Registration Form
- Health Questionnaire
- Immunisation History (This Applies Only To Patient Under Years Of Age)
For your convenience you may print and these forms by clicking the links below and once completed these form must be returned to the practice with the appropriate identification for staff to check. Alternatively you can collect a new patient pack from reception.
